By registering my child for Vacation Bible School at Abundant Life Outreach Center, I acknowledge that I have read and agree to the following: 

• I give permission for my child to participate in all VBS activities. 
• I understand that reasonable precautions will be taken to ensure my child's safety; however, I voluntarily assume any risks associated with participation. 
• I release and hold harmless Abundant Life Outreach Center, its pastors, staff, and volunteers from liability for injuries, accidents, or illnesses that may occur during VBS activities, except as prohibited by law. 
• In the event of a medical emergency, I authorize church representatives to obtain emergency medical treatment for my child if I cannot be reached. 
• I certify that all medical information provided in this registration form is accurate and complete. 
• I understand that photographs and videos may be taken during VBS activities.

I understand that submitting this registration form constitutes my agreement to the VBS waiver, medical authorization, and participation terms.
